What You’ll Do

  • Perform comprehensive patient assessments and develop individualized plans of care.

  • Deliver skilled nursing services in the patient’s home.

  • Coordinate care with physicians and the interdisciplinary healthcare team.

  • Educate patients and caregivers on disease management, medications, and treatment plans.

  • Monitor patient progress and update care plans as needed.

  • Maintain accurate and timely clinical documentation.

  • Support patients and families throughout their healthcare journey while promoting independence and improved quality of life.


Qualifications

  • Graduate of an accredited school of nursing.

  • Current Registered Nurse (RN) license in the state of practice.

  • Minimum of one (1) year of clinical nursing experience preferred.

  • Home Health experience preferred, but not required.

  • Current CPR certification.

  • Valid driver’s license, reliable transportation, and current automobile insurance.
