Hospice Licensed Practical Nurse Continuous Care

Job Summary

Queen City Hospice is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to provide direct patient care in a hospice setting. This role focuses on delivering high-quality, patient-centered care that supports comfort, dignity, and quality of life for patients and their families. The ideal candidate has strong clinical skills, excellent communication abilities, and a passion for end-of-life care.


Key Responsibilities

  • Provide direct patient care in accordance with the State Nurse Practice Act
  • Deliver nursing care based on comprehensive patient assessments and established plan of care
  • Accurately document patient care and services in a timely manner
  • Educate patients and families on diagnosis, care plans, and symptom management
  • Coordinate hospice services with the interdisciplinary care team
  • Communicate patient needs, status changes, and care updates effectively
  • Provide pain and symptom management interventions
  • Monitor and evaluate patient responses to treatments and medications

Required Qualifications

  • Graduate of an accredited Practical Nursing (LPN/LVN) or Vocational Nursing program
  • Current Licensed Practical Nurse (LPN/LVN) license in the state of practice
  • Minimum 3 years of nursing experience
  • Current CPR certification
  • Strong verbal and written communication skills
  • Excellent organizational and time management skills
  • Valid driver’s license and reliable, insured vehicle

Preferred Qualifications

  • Experience in hospice, home health, community health, or medical-surgical nursing
  • Understanding of hospice philosophy and end-of-life care principles
