Position Summary: This position performs assessments and simple nursing procedures in client’s homes, under the guidance of the RN Supervisor, and is specifically responsible for implementation of nursing care based upon the plan developed by a Registered Nurse (RN) in accordance with physician’s orders.
Essential Duties:
- Performs selected nursing procedures, commensurate with experience and education within the scope of LVN/LPN practice, including but not limited to: tracheostomy care, colostomy care, enemas, douches, catheterizations, dressing changes, and phlebotomies.
- Provides patient and family teaching according to plan outlined by the RN.
- Monitors patient's vital signs and makes specific measurements and observations.
- Monitors and records intake and output. Reports significant changes immediately.
- Observes, assesses, and records patient's physical and mental condition and reports any changes.
- Administers medications as ordered by the physician and instructed by the RN. Documents the patient’s file accordingly.
- Attends and participates in multi-disciplinary patient case conferences, staff meetings, and in-service education programs as assigned.
- Works with patient, family, and health care team members effectively to coordinate patient care.
- Cooperates with on-call schedule, which includes weekends and after hour visits, as necessary.
- Maintains a high degree of confidentiality at all times due to access to sensitive information.
- Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the agency.
- Follows all Medicare, Medicaid, and HIPAA regulations and requirements.
- Abides by all regulations, policies, procedures, and standards.
- Performs other duties as assigned.

Position Requirements & Competencies:
- Must be a graduate of an accredited school of practical nursing.
- Must be licensed as a vocational/practical nurse in the state of practice.
- Must have at least one year clinical practical nursing experience— preferably in intensive care, rehabilitation, or medical surgical nursing; home health experience preferred.
- Must be able to make home visits and provide nursing care in the home environment.
- Must be able to complete necessary documentation appropriately, legibly, and in a timely manner.
- Must have interpersonal, verbal, and written communication skills necessary to complete job responsibilities.
- Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
Working conditions: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to sit, talk, hear, and to use hands to finger and handle controls. The employee frequently is required to reach with hands and arms. The employee is occasionally required to stand, walk, bend, twist, reach, and stoop. Specific vision abilities required by this job include close vision and the ability to adjust focus.
Universal precautions and infection control standards must be maintained; there may be unsafe environments in a consumer’s home (e.g. pets). Employee may be exposed to blood and body fluids, household dust, cigarette smoke, needles, and other sharp instruments. May need to use mask, gloves, goggles, or gowns.
Environment: Moderate noise level similar to a typical home environment
