Home Care Service Coordinator

Position Summary: Responsible for scheduling and supervising in-home care workers and clients in a geographic area. Supervisory and/or home care experience preferred.

Hours: Full Time: Monday through Friday 8 am to 5 pm. Plus rotating on call every other week taking afterhours calls; travel within local geographical territory.

Essential Duties:

  • Schedules employees as directed by client's care plan established upon intake.
  • Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence. Provides alternate coverage to ensure the client's care plan is followed and client services are not interrupted.
  • Complete client intake paperwork with routine follow up visits.
  • Monitor systems to ensure staff are clocking in and out for scheduled shifts.
  • Supervises direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company's policies and guidelines.
  • Cross train for new hire process of new employees and ensures all documentation is completed accurately and in a timely manner.

Position Requirements & Competencies:

  • Must have high school diploma or equivalent
  • 1 year of scheduling experience is preferred
  • 1 year home care experience
  • Interpersonal, organizational and communication skills
  • Computer skills including but not limited to Microsoft Word, Microsoft Excel, use of multiple systems
  • Must have reliable transportation
