Entry level Community Care Manager - Deer Park

POSITION SUMMARY: The Client Relationship Manager is responsible for revenue growth and coordinating delivery of services for clients. Responsible for opening new cases and ensuring that care plan is appropriate for level of care needed. Providing support with planning, directing, and organizing the delivery of services in a manner that ensures quality standards consistently meet or exceed the established philosophy, expectations and goals of the company.

Hours: Monday through Friday - 8 am - 4:30 pm.

Location: 21840 W Lake Cook Rd, Deer Park, IL 60010

Salary: $50,000 - $58,000

ESSENTIAL DUTIES:

  • Takes initiative to create and implement programs to increase revenue and client population.
  • Responsible for the care coordination of clients
  • Establishes and maintains relationships with referral sources
  • Monitor care plans to ensure client needs continue to be met.
  • Coordinate client weekly billing and transmittal.
  • Performs supervisory visits.
  • Meet or exceed financial goals in regards to growing the busines.
  • Responsible for scheduling staffing needs for all convenience services
  • Conducts caregiver orientation/training.
  • Coordination of client and caregiver schedules, approval of time slips and entering corrections/changes in scheduling software.
  • Caregiver coaching and counseling as required.
  • Addresses client and/or caregiver concerns and complaints in a timely manner. Performs investigation as needed.
  • Ensures timely submission of all required caregiver paperwork
  • Maintains regular, predictable, consistent attendance and is flexible to meet the needs of the department.
  • Follows all HIPAA regulations and requirements.
  • Abides by all regulations, policies, procedures, and standards.
  • Performs other duties as assigned.

POSITION REQUIREMENTS & COMPETENCIES:

  • Must have a High School Diploma or equivalent
  • Two years health care experience preferred.
  • Certified Nurse’s Aide in good standing with the state is preferred.
  • Strong communication skills and interpersonal skills
  • Ability to work independently.
  • Valid driver’s license and proof of insurance is required.
  • Proficient in Microsoft Office Suit.
