Home Care Service Coordinator

Position Summary: The Service Coordinator position is responsible for managing the day-to-day operations of the office. The tasks include anything from coordinating client care within the client's authorization, hiring/onboarding new employees, addressing daily visits within the compliance standards, conducting new client intakes, creating schedules, completing client incident reports, etc. This is an ideal position for someone who enjoys working with people, has compassion for both clients and staff, and has patience along with leadership experience. If you enjoy managing people and paperwork and have outstanding communication and time management skills - this would be a wonderful opportunity for you!

Hours: Full Time: Monday through Friday 8 am to 4 pm.

Hourly Rate: $14.35

Essential Duties:

  • Schedules employees as directed by client's care plan established upon intake.
  • Processes patient authorizations and communicate with central admissions, enter reauthorizations into client record and ensure chart preparation for all new clients.
  • Creates work schedules by entering schedules into the system, manages changes to client schedules due to client request, illness, vacation or leaves of absence. Provides alternate coverage to ensure the client's care plan is followed and client services are not interrupted.
  • Duties will include traveling into the community multiple times a week for marketing, recruiting and client support.
  • Provide Client Care. Able to cover a critical client who does not have a backup in place when an aide is unavailable.
  • Supervises direct service employees by setting expectations for attendance, performance and conduct by holding employees accountable to the company's policies and guidelines.
  • Assists with the new hire process for all new employees and ensures all documentation is completed accurately and in a timely manner.

Position Requirements & Competencies:

  • Must have high school diploma or equivalent.
  • Interpersonal, organizational and communication skills.
  • Boots on the ground recruiting and covering shifts in the field as needed.
  • Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
  • Must have reliable transportation (Driver's License, Auto Insurance, Vehicle).
