What You’ll Do

  • Evaluate patients and develop individualized treatment plans based on physician orders.
  • Provide skilled physical therapy services in the patient’s home.
  • Help patients improve mobility, strength, balance, and overall functional independence.
  • Educate patients, families, and caregivers on treatment plans, home exercises, and safety strategies.
  • Recommend adaptive equipment and home modifications to promote safety and independence.
  • Collaborate with physicians and the interdisciplinary care team to coordinate patient care.
  • Accurately document evaluations, progress notes, and patient outcomes in a timely manner.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations and company policy.
  • Encourage and support patients throughout their rehabilitation journey.


Qualifications

  • Graduate of an accredited Physical Therapy program approved by the American Physical Therapy Association (APTA).
  • Current Physical Therapist (PT) license in the state of practice.
  • Home Health experience preferred, but not required.
  • Current CPR certification.
  • Valid driver’s license, reliable transportation, and current automobile insurance.
