Qualifications:

• Experience working in a community-based setting for at least 1 to 2 years preferred.
• Experience working with patients with dementia preferred.
• Maintains a current CPR certification
• Strong communication skills and interpersonal skills
• A valid driver’s license and proof of insurance is required.
• Good organizational skills to handle multiple priorities while remaining professional and calm.
• Ability to work with many diverse people, including children and teenagers.
• Effective telephone skills.
• Strong level of confidentiality due to the sensitivity of materials and information handled.
• Ability to make suggestions on workflow or system efficiency and effectiveness.
• Ability to work independently and be self-directed and flexible.
• Ability to prioritize.
• Ability to perform functions with minimal supervision.
• Ability to work at a high-volume level of accuracy.
• Written and oral fluency in English and Spanish is preferred.
• Basic computer skills required, electronic medical record (EMR) experience preferred.
• Understand the community served, community connectedness.
• Good communication skills, such as listening well, and using language appropriately.
• Ability and willingness to provide emotional support, encouragement and motivation to patients.

HOURS: Monday-Friday 9AM-5PM

Salary: $25 - $35 Hourly - Offer Based on Years of Experience

As a member of the Palliative Care team, the Community Health Worker (CHW) is responsible for helping patients with dementia and their families to navigate and access community services, other health resources, and adopt healthy behaviors supporting their conditions. The CHW supports providers and the clinical care team through an integrated approach to patient care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. CHW provides social support and informal counseling, advocates for individuals and community health needs.

What You’ll Do:

• Engage with community members, potentially through home visits or community events, to identify individuals who could benefit from the GUIDE program.
• Maintain compliance with GUIDE program requires as outlined by CMS.
• Assists in representation of supportive services and GUIDE program at community events.
• Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.
• Work closely with medical providers to help ensure that patients have comprehensive and coordinated care plans.
• Coordinate care among various providers, ensuring that individuals receive comprehensive and integrated services
• Provide ongoing follow-up, basic motivational interviewing, and goal setting with patients/families.
• Help patients connect with transportation resources and appointment scheduling.
• Connect in person visits to provide support to patients and families affected by dementia.
• Possesses workable knowledge of dementia and home care services.
• Assist with the development and implementation of professional and community education programs.
• Assists with, participates in, and tracks the outreach initiatives for the agency
• Provide assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs.
• Act as a patient advocate and liaison between the patient/family and community service agencies.
• Follow – up with patients via phone calls, home visits and visits to other settings where patients can be found.
• Responsible for providing consistent communication to clinical care team to evaluate patient/family status, ensuring that provided information, and reports clearly describe progress.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• Attend regular staff meetings, training, and other meetings, as requested. Maintains collaborative relationships with organization personnel to support patient care.
• Actively participates in quality assessment performance improvement teams and activities.
• Manage assigned caseload of patients.
• Maintain HIPAA compliance at all times.
• Performs other duties as assigned.
