Licensed Practical Nurse – LPN Hospice Care

Department: Clinical - Nurses
Location: Mason, OH

LPN – Hospice Care

Queen City Hospice – Southwest Ohio
Schedule: 3x 12-hour shifts/week, including alternating weekends

What You’ll Do:

  • Deliver direct patient care per State Nurse Practice Act

  • Conduct and follow up on comprehensive assessments and care plans

  • Accurately document patient services in a timely and detailed manner

  • Provide education and support to patients and families/caregivers

  • Coordinate hospice services and communicate patient needs to the interdisciplinary team

  • Manage pain and symptom control, evaluate treatment effectiveness


Qualifications:

  • Graduate of an accredited practical/vocational nursing program

  • 3+ years of nursing experience (hospice, community health, or med/surg preferred)

  • Current LPN/LVN license in the state of practice

  • CPR certification (active)

  • Strong verbal, written, and organizational skills

  • Valid driver’s license and auto insurance as per state requirements

  • Understanding of hospice philosophy and the needs of the terminally ill
