Schedule: Monday–Friday | 8:00 AM – 5:00 PM

Provide personal care for patients (baths, grooming, oral hygiene, etc.)
Assist with dressing and undressing, as assigned
Ensure patient safety and use of medical equipment properly (e.g., O2, side rails)
Help with meal planning and preparation; shop and feed patients as needed
Take and record vital signs (temp, pulse, respiration, blood pressure) as instructed
Monitor and report skin condition and any signs of breakdown
Communicate significant changes in patient condition to the Case Manager
Provide respite care to relieve family/caregivers when appropriate
Be mindful of family dynamics and interpersonal needs in the home
At least 16 years old
State-certified nurse aide (STNA/CNA)
Current CPR certification
Listed in the state aide registry with a clear record
Comfortable providing end-of-life care and understanding of hospice philosophy
