Schedule
Three 12-hour shifts per week 7 p.m.7 a.m.
Includes rotating weekends
Two holidays per year

What Youll Do
Provide direct patient care in accordance with the State Nurse Practice Act
Implement nursing interventions based on comprehensive assessments and individualized plans of care
Document patient services accurately and in a timely manner
Educate patients and caregivers on diagnoses, care plans, and symptom management
Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
Manage pain and symptoms effectively and evaluate patient response
Support patients and families with compassion, professionalism, and respect
What You Bring
Graduate of an accredited practical or vocational nursing program
Current LPN/LVN license in the state of practice
Three years of nursing experience (community health, hospice, or med/surg preferred)
Current CPR certification
Strong communication and organizational skills
Valid drivers license and insured, reliable vehicle
Understanding of the hospice philosophy and the needs of the terminally ill
