What You’ll Do

In this RN liaison role, you’ll combine clinical expertise with relationship-building:

• Perform hospice eligibility assessments and bedside evaluations
• Coordinate hospice admissions and support patients/families through the process
• Partner with physicians, hospital case managers, and care teams
• Educate referral sources on hospice services and criteria
• Ensure smooth, timely transitions into hospice care
• Maintain strong relationships across your assigned territory

Qualifications

• Active Illinois RN license
• Graduate of an accredited nursing program
• 1+ year of RN experience (hospice, palliative care, case management, or hospital experience preferred)
• Current CPR and First Aid certification
• Strong clinical judgment and communication skills
• Ability to travel within territory (50%+)
• Valid driver’s license and insurance
