Position Summary
The Hospice On Call RN (Night Shift) provides after-hours clinical support, managing patient and family needs through telephonic triage, care coordination, and in-person visits when needed. This role ensures that patients receive timely symptom management, compassionate support, and continuity of care outside traditional business hours.
Key Responsibilities
- Provide after-hours support for hospice patients and families
- Assess patient needs and determine appropriate interventions, including symptom management and care plan updates
- Coordinate and delegate nursing visits and services to field staff as needed
- Evaluate physical, psychological, social, and spiritual needs of patients
- Communicate with physicians, interdisciplinary team members, and external partners to ensure continuity of care
- Perform clinical pronouncement of death and provide compassionate family support
- Maintain accurate, timely clinical documentation, care plans, and progress notes
- Ensure compliance with hospice regulations and standards of care
Required Qualifications
- Graduate of an accredited Registered Nursing (RN) program
- Current RN license in the state of Ohio
- Minimum 2 years of clinical nursing experience (hospice, home health, ER, ICU, or med-surg preferred)
- Strong clinical judgment, triage, and decision-making skills
- Ability to manage high-emotion situations with compassion and professionalism
- Excellent communication and organizational skills
- Current BLS certification
- Valid driver’s license and proof of insurance
