What Youll Do
Triage after-hours patient and family calls, determining needed services such as assessments, symptom management, or care plan updates
Delegate nursing visits to appropriate team members based on patient needs
Assess physical, emotional, social, and spiritual factors impacting patient and family well-being
Coordinate interventions with the interdisciplinary team
Communicate with attending physicians, team members, and partner agencies to ensure seamless care
Perform clinical pronouncements of death and provide compassionate support to families
Maintain accurate, timely documentation, care plans, and progress notes
What You Bring
Graduate of an accredited registered nursing program
Current RN license in the state of practice
Minimum 2 years of nursing experience
Strong clinical judgment and nursing skills
Ability to support families during emotional and stressful moments
Current CPR certification
Valid drivers license and proof of insurance
