Requirements:

· Valid LPN or RN license through the State of Ohio without disciplinary actions

· Must be able to pass fingerprint & background check

· Reliable transportation (a personal vehicle, a driver's license and car insurance)

· Assessing care plan compliance

· Evaluating patient needs

· Infection control and safety checks

· Reporting changes
